This is the official one:
ISO - International Organization for Standardization

It doesn't include the dialcodes though. The IP ranges are somewhat more complex because of the subnets. There are some sites that do it (I've been working since 0230 on spreadsheets so I can't remember them due to a lack of lucidity ). I have my own software here for compiling IP ranges/Country tables though.
