Default PHP form to capture variables and then email?

Does anyone have any recommendations for a cheap and cheerful php form that will retrieve variables from a URL and send them in an email.

Nothing too fancy, it's actually just to work with the response URL that Realex sends back to the merchant site after a transaction has been completed. I need to capture this info and send it on to the merchant's email address so they have a record of the transaction.
