I've heard they only use them to tweak their algorithms, so that they will catch them automagically. Sadly I don't have a source to back this up.

The witch hunts over at DP can give an indication on how long it can take. I believe the more people reporting on the violator the faster the case will be dealt with.

I was doing some competitor analysis a few months ago, and found I guy labelling his adsense "Click our sponsors to help us out", in bright red. I emailed his details to google and within a week, his adsense was gone...

My main concern (at the moment) is adwords advertisers breaking the TOS eg. advertising things they don't actually sell or making price promises that are unrealistic

I've had it acted on relatively quickly through an account manager, in my case an Ad was advertising an offer which was unavailable on their website.
