Haven't used mine yet but you should have received an email with the code in it ?

Also I'll assume you'll have to give a credit card to facebook to make use of their 100Dollars ... probably a way of enticing people to use the 100$ for one and to stay using their service after.

yes you have to give details before they mail you any code, it says it wont charge anything to card but not sure
