Not always, I'd say about 70% (rough guess) of the time here.

For larger scale projects I think it's beneficial to meet the client and build up a relationship.
Have done lots of small scale projects and never meet clients, usually it's pretty easy to spot the time wasters and those that will give you headaches.
I meet every single one of them. Maybe just once, but I always build a relationship before I even think of accepting a project.
I turn down 40-50% of projects because I do computer stuff mostly as a hobby(i can afford to refuse them), and in most cases, it's because there's something big wrong with the client, but I always point in the right direction.
I usually always meet up as well, think it's beneficial for both parties to get to know each other better then you can do by phone.
Depends, if they are in the same city/country as me, and would depend on the size of the project whether there would be budget for me to meet them personally. If I can meet them I will, if not, I would video conference / phone them.

Have only ever met one of my clients... And that didn't go well so I tend not to meet them if I can.
I don't mind meeting the client to develop the relationship but don't particularly like having to meet them so that they can convey what they want. Most meetings tend to be a waste of time.
theoretically in some projects u may need to meet the client many times in many phases of these projects as a software engineering view ...
actually i myself believe in this , as most of the project's time is wasted in arguing with the ur client ... but in the other hand u find it difficult to meet with him every time u seek for him ... (specially with the design issues!)
thats just my point of view no more !
I would say that I meet my clients about 50% of the time depending on the project. Some of my clients like to have the option to meet me
