    Yea, this happened to a few of my site verifications. I thought I may have logged in with another account but nope.

    Only happened to sites verified by meta tag though.

    yes it happened to me as well few times. generated a new verification meta tag code, paste it into you page and ask google to verify it again.
    That worked for me.
